Enrollment Information

Titan families,

\r\n

As you know, we are in the middle of a unique and sometimes challenging season. The COVID-19 pandemic has caused us to look at creative means to deliver meaningful instruction to students in the 2020-2021 school year.\r\nNo Mid-Del school sites are permitting entrance to any parents or students at this time. Because of this, we will be doing enrollment electronically. You will be completing the information below and emailing everything back to cahs@mid-del.net.

\r\n

**Returning student enrollment information for the 2020-2021 school year.\r\n• We mailed out each student’s current information sheet. Please use this sheet to cross out any old or incorrect information and replace with the correct information to the right and sign.

\r\n

• If you have any questions about the paperwork that was sent home, please do not hesitate to call for help 405-739-1726. This will help prevent a delay in your child’s enrollment.

\r\n

• Please attach your (2) current utilities as proof of residency in order to verify your residence\r\n(Ex. Current gas, water and/or electric bills with a due date within 30 days)

\r\n

• All enrollment information and forms sent home need to be emailed to cahs@mid-del.net by Friday, August 7th. If you need to deliver the paperwork to the school you can do that too. Simply buzz the front office between 8 am-3 pm (Monday-Friday) and someone will collect it from you.

\r\n

**Resident Affidavit (RAs) MUST enroll at the Central Enrollment Center.\r\nPlease see information below for how to make your appointment.

As school has been recently changed to start August 24th, we want to make sure that you are aware of a few key procedures. We feel that each of the secondary and safety procedures listed on the “Return to School Plan” are important.
