Guardianship Information and Requirements
Natural/Adoptive Parent
Must provide the student birth certificate (an original, certified copy is required for all Pre-K through 1st grade students, a non-certified copy, hospital issued certificate, passport, etc. is acceptable for 2nd grade through 12th grade students)
Must provide a photo I.D. (state issued driver’s license, state I.D., military I.D. or green card)
If divorced, must provide the divorce decree including any and all custody determinations (if natural parents were never married but both are listed on the birth certificate, either parent may enroll the student IF there is no existing custody documentation)
If a step-parent, must provide the marriage license showing marriage to the custodial parent along with all other documentation
Legal Guardian
Must provide the student birth certificate (an original, certified copy is required for all Pre-K through 1st grade students, a non-certified copy, hospital issued certificate, passport, etc. is acceptable for 2nd grade through 12th grade students)
Must provide a photo I.D. (state issued driver’s license, state I.D., military I.D. or other government issued I.D.)
Must provide court ordered placement documents (court appointment of legal guardianship based on necessity that has been signed by a judge or foster placement documents) NOTARIZED STATEMENTS WILL NOT BE ACCEPTED
Guardian for a Transitioning Military Child
Must provide the student birth certificate (an original, certified copy is required for all Pre-K through 1st grade students, a non-certified copy, hospital issued certificate, passport, etc. is acceptable for 2nd grade through 12th grade students)
Must provide a photo I.D. (state issued driver’s license, state I.D., military I.D. or green card)
Must provide the fully executed Military Power of Attorney
Must provide a copy of the parent/legal guardian’s orders that are effective within 30 days of planned enrollment
